PRIVACY STATEMENT

To provide for your care at our practice we need to collect and keep information about you and your health on our records. At Watermills Family Practice maintaining patient confidentiality is a priority – our medical records are computerised, password- protected, and information is safely backed up. We capture only information that is necessary for your care. All staff members have signed a confidentiality agreement that states they will not disclose any medical or personal information.  Both personal and medical information is treated with utmost respect complying with our duties under the Data Protection Acts 1988 and 2003. Patient’s medical or personal information will not be disclosed to a third party unless previously agreed in writing by the doctor and patient, except for referral letters where permission is deemed implicit.

If you decide to change your doctor, a copy of your electronic record will be made available to your new doctor on your request.

We will maintain a record in our practice for medico-legal purposes for an appropriate period.

​

For patients under 16, medical information may be disclosed to the child’s guardian.
